Hello everyone…new here to the message board and have a few questions. I am currently on acid reflux baby #2. With the first baby, Trenton, he spit up constantly and when i say spit up, i mean from one bottle to the next. He cried constantly…all the typicals. We tried zantac first, then another, i think reglan, and then finally ended up on prevacid which did us wonders. We also in the mean time switched his formula 13 times and added tons of cereal to his bottle. Baby #2 now 8 weeks started having symptoms at 3.5 weeks. I really thought we were gonna get by without it this time. I am pretty sure he also has a MSPI as when i eat or drink dairy products, he seems irritated. I am strictly expressing breastmilk after all kinds of breastfeeding problems. The doctor first put him on prevacid since he said that it seems to work the best for most babies. It seemed like he did fine on it for a few days and then it got bad again. He got very fussy again and irritated and i noticed him gagging and swallowing hard after feedings when the acid would come up as he has silent reflux. He was taking half of the solutab. The dr had us call back in a week and when i did i told them of the experiences and they switched us to zantac, 1.5ml 2x a day. He has a real hard time taking this and i find that he ends up spitting up after giving it to him. I also still notice him gagging and swallowing hard. I feel like maybe i didn’t give the prevacid enough time. If you have any help or suggestions, please give your advice. Things i have read make me pesimistic (sp?) about the zantac and things getting bad again soon. I’d appreciate anyone’s experiences and advice.
Have a great day!November 11, 2008 at 3:58 pm #58553AnonymousInactiveIf it is the commercial preparation of zANTAC, (mint flavour) see if you can switch to a pharmacy compounded zantac instead. The commercial prep has alcohol and mint two big offenders to aggravating reflux in infants.
Not sure why the pediatrician had you go from Prev to Zantac, that`s like going backwards in treatment. IMHO Often babies will develop a immunity to an H2 blocker:
Medicines stop working
The medical term for this is
tachyphilaxis. It means resistance to medicine. With H2 Receptor
Blockers (Zantac, Tagamet, Pepcid, Axid), the stomach simply develops
more receptors to compensate for the blocked receptors that signal when
to produce acid. Switching brands may help an amazing amount or you may
need to use a different type of medicine.from:http://www.reflux.org/reflux/webdoc01.nsf/(vwWebPage)/Vascillate.htm?OpenDocument
I`m guessing the prevacid dose was too low and too infrequent. PPIs are often underdosed in infants and to see full benefits you need the right dose at the right frequency over a longer period of time (two weeks seems the average before cosistant relief is established). see:http://www.marci-kids.com/dosing.html

November 11, 2008 at 5:06 pm #58559hellbenntKeymastercheck the dose of prevacid http://www.marci-kids.com
and check the ‘FORM’ of prevcid Prevacid 101(last post on the page): https://www.infantreflux.org/forum/forum_posts.asp?TID=1936&PN=0&TPN=1in the meantime, you can give the zantac AND the prevacid making sure to space the doses apart 4 hrs.the max dose for zantac is 10mgs per kilo so you may have room to increase the dose:Zantac dosing- my/other’s question (has axid & pepcid dose, too): https://www.infantreflux.org/forum/forum_posts.asp?TID=9539&PID=82914#82914
November 11, 2008 at 5:52 pm #58564AnonymousInactiveOk, i checked out the posts you gave me. First off, my doctor never said one thing to me about waiting a half hour to hour after giving him the medicine before i feed him. Why do you think this is? Second, my doctor was having me take one tablet and melt it in 4 mls of water and then give him 2 mls. Which obviously amounts to half of the medicine. I did my research at marci-kids.com and it says that he can be taking 3x that much. Holy cow!!! So, now my question to you is how do i go to my doctor with this information without offending him, because i really like him…i just think he didn’t have the right info. My 1st son never took more than one tablet a day all the way up to a year old and he only took it once a day. Also, how long do you recommend staying on the zantac to wait for the prevacid to “kick-in”?
November 11, 2008 at 6:01 pm #58565hellbenntKeymasterstay on the zantac for a good 2 weeks, giving it 4 hrs seperate from the prevacid
you can keep baby on zantac and prevacid for longer- you might need to, until you get the prevacid dose ‘figured out.’I wouldn’t give the solutabs how the dr said to- many just hold it in alongside inside of baby’s cheek and wash down w/ a syringe of water- htere’s a link from prevacid 101 w/ tips how to administer it…babies have very fast metabolisms so they speed through the meds- that’s why most find better success dosing baby 2 or even 3 times a day. if it’s given all at once, baby zips through it and there isn’t any left in the body to hold her/him over a full 24 hrs…FAX FAX FAX the dr
dont wait for a call…
start off w/ a THANK YOU
keep it short & sweet
stick to the facts (no feelings and no rambling emotions)
state current meds, doses, symptoms
make request- flat out- short & sweet
THANK AGAIN
follow up by calling office to say fax was sent
call again later to discuss requests in fax, ie: have you called in the increased prescription yet? THANK YOU
