No, it’s ok to mix the mylanta with the Zegerid. In fact, were you using the 40 mg packs, that’s exactly what they want you to do.

And yes, it’s ok to mix old and new. I would maybe let the refrigerated one warm up a bit and shake it up, so that it’s all mixed well before you add it.
