My son Jack is 4 months old and has silent reflux. We are going for his 4-month check up on Monday, but I am sure he weighs somewhere around 15 lbs. He had an NG tube in place at 1 month due to poor feeding secondary to reflux, but is now weaned off of the tube. He has been doing well on Prevacid 7.5 mg three times a day per MARCI-kids dosing recommendations, but he got an double ear infection and was refusing bottles. The ear infections have cleared, but they exacerbated his reflux. On 2/5 we realized that maybe his bottle refusal was do to reflux. The prevacid compound that we had from the pharmacy was older, so we thought we would get fresh made to try to help him take his bottles better. We also gave him Mylanta. The Mylanta really helped but the fresh batch of Prevacid doesn’t have him as well controlled as before. I called my doc to see if he would go higher on the dose and he said no. I am willing to try to use OTC prevacid in Pedialyte or make a Zegerid compound but don’t know where to start. Which should I try first? Upping the Prevacid dose on my own with OTC or go right for the Zegerid compound? Also what would the right dose be for the Zegerid compound for my son?
I have emailed Dr P but haven’t heard back. I’m getting desperate as he is having poor feedings and is so uncomfortable. We don’t want to have to go back to the tube. Any help would really be appreciated.Thanks!February 9, 2012 at 6:05 pm #69803hellbenntKeymasterIf you decide to do OTC prevacid, you must buy capsules & open them up & you administer the beads inside the capsule. You must give on an empty stomach, followed by a ‘meal’ 30 minutes later. When you administer the beads you need to give in a tsp of applesauce or in some pediatlyte:

zegerid has the drug prilosec in it and it has buffers, so that it can be given w/ food. Otherwise, you must give a PPI on an empty stomach, followed by a ‘meal’ 30 minutes later. Examples of situations where you have to follow the ’30 minute rule:’ if you use prevacid solutabs or, if you use either prevacid or prilosec capsules (you would open them up and give the beads inside the capsule- you would give on an empty stomach and administer the beads w/ 1/2 tsp of applesauce & then you would follow w/ a ‘meal’ 30 minutes later).
why should you care about zegerid? because you can give the medicine at any time, even with food, and you don’t have to worry about timing it around an empty stomach.
are there other options?
yes, you can buy over the counter capsules and administer the beads inside the capsule.
any other options?
you can try and TELL the pharmacist EXACTLY how to compound (make into a liquid) your prevacid or prilosec. However, for whatever reasons, it can be difficult to get a high enough dose prescribed and, for whatever reasons, pharmacist insist they’re doing it correctly their way and they don’t take kindly to compounding the way you or your doctor tells them to.

If you make your own liquid, w/ OTC zegerid & OTC prilosec, (link above)then: This makes a concentration of 2 mg/ml and 3.5mL is the minimum volume to give each dose.

Thank you for the information. We tried the OTC Prevacid this afternoon, but it was hard to divide up the beads correctly to be sure we were giving him the right dose. Any tips? According to Marci-kids, we are supposed to give him 10 mg three times a day. The capsules are 15 mg and I probably gave him close to all of the capsule this afternoon. He still had a reflux flare at 4 pm-ugh! I am going to try to make the Zegerid mixture. The Omeprazole beads don’t dissolve in the mixture-right? You just have to be sure to shake up the liquid to evenly disperse them prior to dosing? Also, shoud I follow Marci-kids for Prilosec dosing? If so, I think it is still 10 mg three times a day-right?Thanks so much for all of your help and please wish up luck. and, there will be break-through acid, especially in severe cases (my son’s, for example…)the beads actually might dissolve eventually? He had a painful crying reflux spell at 4 pm so I know we are not controlling it right now. Hoping that the Zegerid mixture helps tonight. February 10, 2012 at 8:18 am #69812AnonymousInactiveWill your doctor prescribe you Zegerid? Mine did, they were powder packets that were really easy to mix and give. It worked well for us. Maybe you can get your doctor to just change you over.

February 10, 2012 at 1:17 pm #69816AnonymousInactiveOurs wrote for the 40 mg packet and we mixed it according to marci kids…cannot remember the exact way, but it was a mix of water and cherry mylanta, then we gave 4ml three times a day. Since it is a different med it will probably have a different mg anyway.
Your doctor should know how to do it, or maybe Laura can pipe in and help with the mixing/dosing. Sorry..it’s been awhile.February 10, 2012 at 2:48 pm #69818AnonymousInactiveThank you so much! We tried the Zegerid mixture and while it was nice to be able to give it to him with the bottle, all of the Mylanta made him very gassy and fussy during feedings. We stuck with Prevacid and adjusted the dose up based on his new weight according to MARCI-kids. We gave the OTC Prevacid with some Pedialyte. He’s definitely on the mend. 🙂
