Ok, I need some advice. My DS is on 30 mg. of Prevacid. This is higher than the recommended Marci-kids dose which is 22.5 mg. I know this is an unusual concern given that most fight to get a higher dose but the Ped. GI said not to split the pill in half. So, I am just wondering what others think about the dose. Is Prevacid like Zantac in that it is weight sensitive and will no longer work as he gets bigger? I guess I am concerned that I have no where to go from here if this stops working. Could I be doing any harm by giving such a high dose…he is about 12 pounds and 12 weeks.
July 15, 2008 at 9:50 am #53966AnonymousInactiveAll of the research on marcikids suggest that it is *nearly* impossible to overdose a PPI because it ONLY targets the acid producing cells in the body. Have you visited their website yet? They have a dosing calculator that you can play around with…put in an older age, higher weight etc…and it will give you an idea of when he might outgrow his dose. My guess is that it won’t be for several months. While PPIs ARE weight sensitive, they aren’t quite as sensitive as zantac.
I know what you mean about hoping this ‘last effort’ will work. After trying zantac and compounded omeprazole…I knew that zegerid (another PPI) was our last hope. I was soooooo nervous it would not be the magic cure all. For Both my boys (although this is rare) couldn’t tolerate the solutabs as they contain very small trace amounts of aspertime and lactose in them, but most babies here that are on the solutabs do fabulous, so I think that is rare. but I think I would def split the dose……

If the acid is being produced all day-which it is-it only makes sense, no??? I’m trying to learn right along with you. How do you give him a solutab???
July 15, 2008 at 10:52 am #53978AnonymousInactiveAnn Marie–OK, so what do you mean by split the dose? I give 15 mg. in the a.m. and 15 mg. in the pm. I give on an empty stomach and follow-up with a feed 30-60 min. later. Is there a better method? Should I cut the pill and only give 22.5 mg? If so, what times should I give?
Kate–I use the solutabs. I started by putting it in a syringe and drawing up water but that wasn’t the best for me b/c too much of the pill got stuck in the syringe. What I do now (without any problems) is dissolve the pill on a spoon with a little bit of water. I then just put the spoon in his mouth and he takes it all down–no problem. I find that giving him the meds. is very easy.July 15, 2008 at 10:56 am #53979AnonymousInactiveLisa, thank you. I’m at the very least getting my baby solutabs today. Dr. P just told me that my “type” of Prevacid is only effective for 5 days. You could totally email him and ask him or one of his assistants your questions. they are WONDERFUL!
July 15, 2008 at 12:12 pm #53990hellbenntKeymasterlisa, if you’re more comfortable with the lesser dose (still the marci-kids dose!), then split the pill. put the remainder in some foil and put in the refrigerator. it’ll be fine.
giving in the am & pm is fine! if you have zantac (or axid or pepcid or tagament) you can give that, too! give it 4 hours seperate from the prevacid. so, if you give the solutab at 6pm then you can give the other med at 10pm (or later) if baby needs it.
July 15, 2008 at 3:05 pm #54007AnonymousInactiveSplitting the pill is perfectly fine to do, and I would go with the 30 mg since overdosing is really not a concern. And I would probably do as you are doing and give 15 mg a.m. and 15 mg p.m, but you could also try 3 doses of roughly 10 mg (divide as best you can). I found that my son needed a little more than what marci-kids recommended; he was on 30 mg, divided into 2 doses of 15 mg because when we tried to divide it into 3 doses of roughly 10 mg, he got very little relief — he needed at 15 mg at one time to knock out the pain. Even so, every day by the afternoon he was in pain again. We gave Zantac then but it did very little — I would have gladly given him 45 mg of Prevacid/day if I could have — that’s how little I would worry about overdosing.
Oh, and Prevacid is not like Zantac as far as being weight sensitive. In fact, as babies grow and their metabolisms slow down a bit, they need less medicine PER KG of weight. For us, this was proven by the fact that Nathan’s a.m. dose of 15 mg did not last til the afternoon when he was younger, but as he grew he didn’t speed through the medicine as fast, and the same a.m. dose starting lasting longer, all the way til the p.m. dose, finally giving him all-day relief (this was when he was about 7-8 months old). Like Amy said, you can play around with the dosing calculator on marci-kids and see what I mean.Hope this helps! Good luck!July 15, 2008 at 3:59 pm #54010AnonymousInactivePaula and Laura got to this before I did… Yes, split and give 15 am and 15 pm, or a bit less, but I think 15 at a time is OK. I would follow each dose up with a feed thirty minutes later…. HOpefully that will give you longer lasting relief….
