I saw it w/ my own eyes!on page 2 of this thread it’s now posted
I asked Dr. P and he said that he prefers (1) 20mg OTC Zegerid capsule plus (1) 20 mg omep capsule. plus 19 ml MCS… because of the amt of bicarb.
Shake and refrigerate, good for one week. To help with excess gas from the buffering in Zegerid, you can add Mylicon drops in the mixture and some in the bottle.
3.5mL is the minimum volume to give each dose.Dr P also says: “19 mls is plenty, but 13mL and the remainder in water is fine too.”
I posted to ask if this means 13mL of mcs & 6mL of water (I think that’s what he meant?)
basically you use enough calcium carbonate in whatever form (mcs, tums, cc powder [NOW brand]) to equal 1,000-1,500 mgs of calcium carbonate and add enough water to make it 19mLs.
This makes a concentration of 2 mg/ml
and 3.5mL is the minimum volume to give each dose. whew!SO: what I gather and, this is just ME here…
The cvs version of mcs has, Per Teaspoon (5mL): Calcium Carbonate 400 mg
[Preferred Plus brand liquid cherry antacid has, Per Teaspoon (5mL): Calcium Carbonate 400 mg]
so: 13mLs of cvs brand mcs has approx ~ 1,000mgs of calcium carbonate
so you find the equivalent amount of calcium carbonate and add enough water to make it 19mLs…CC powder (NOW brand) = (CHECK YOUR BOTTLE! This link = 600 mg of calcium (keep reading!!) in 1/2 tsp. And 600mg of calcium is equal to 1500mg of calcium carbonate! So you just use 1/2 tsp, if that’s ‘all’ you’re using to mix this up. You can also mix a combo of things, but keep reading!)
(TUMS info, below, updated July 2015, from TUMS website)
1 TUMS Ultra 1000 = 1,000mg of Calcium Carbonate (no milk or soy protein)
1 TUMS EXTRA = 750mg of Calcium Carbonate (no milk or soy protein)
1 TUMS SMOOTHIE = 750mgs of Calcium Carbonate (no milk and DO have soy…EXCEPT, the PEPPERMINT SMOOTHIES do NOT have soy)
1 TUMS REGULAR STRENGTH = 500mgs of Calcium Carbonate (no milk or soy protein)
1 TUMS KIDS = 750mg of Calcium Carbonate (no milk but do contain soy; the peppermint does not have milk or soy)
1 TUMS Chewy Delights = 1,177mg of Calcium Carbonate (have both milk AND soy)

TIPS:
name brand Prilosec tablets are dairy free and they disolve when cut in half
With OTC omeprazole capsules: they come w/ a little tablet. you don’t use the tablet. just the beads.
The little tablet inside the capsule can be discarded (it is there because Prilosec OTC is omeprazole magnesium), instead of plain omeprazole – (Prilosec prescription is plain omeprazole). So in order for the omeprazole OTC products to get FDA approval, they had to add a small amount of magnesium, so it would have some magnesium in it. The magnesium does not do anything, hence the little tablet can be thrown away.TIPS:
Get capsule Omeprazole if possible, if not use a coffee grinder to make the tablet a powder
Do NOT make more than a 14 day supply, as the longer the solution is sitting the less potent it becomes
If making a 14 day supply and you start to notice the med isnt helping as much, you should make less of the solution as it is probably breaking down
Try adding Splenda, Truvia, sugar or Chocolate syrup (if safe for your child), coffee sweeteners (not sugar free), simple syrup to sweeten the mixtures (give with each dose just in case adding to whole mixture compromises the solution).
After mixing, let solution sit in fridge for 2-3 hrs before giving as this will give the solution time to mix completely
If gas is an issue, try using the 4mg/ml mixture (see below)
Zegerid capsules don’t open easily, so I used a push pin to poke a hole in the top of each and squished out the powder
Grind up TUMS in coffee grinder to make a powder
Give gas drops with each dose to help with excess gas problems~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~~

10ml WaterA 4mg/ml Big Batch Recipe:
We just weaned my lo off Zegerid at 11 mos. was a miracle for us. We did the 4mg/ml and it worked great. His dose was always around 2.5 ml. My recipe for a big batch was:
14 Prevacid- generic tablets
14 Zegerid
7 tums smoothies
7 tsp calcium carbonate
133 ml water/simple syrup combo
Grape flavoring
I would dissolve the Prevacid in 20 ml water. I did that in a small bowl and would break them up with the back of a spoon. I then opened the Zegerid and put it into one of the Medela bottles. Then crushed the tums, added them and the cc. Then added the Prevacid to this mixture. Then I measured the remaining 113 ml liquid at about 2/3 simple syrup, remainder water. I added about one dropper full of grape flavoring.
This would last about three weeks at 2x a day dosage. This is just what worked for us- please tAlk to your dr before using any recipe found in the Internet! All components purchased on Amazon. I used a pill grinder for the tums. Neither a coffee grinder or pill crusher ever worked well for the Prevacid. I used the tablets not capsules- generic brand.November 16, 2011 at 6:01 pm #69377hellbenntKeymasterluckyjill22 wrote: I asked Dr. P and he said that he prefers (1) 20mg OTC Zegerid capsule plus (1) 20 mg omep capsule. plus 19 ml MCS… because of the amt of bicarb.
Open the zegerid capsules to get out the powder
Cut the omep pill in half and then put both halves in the mixture so it can dissolve.
Some crush the pills, because they find that cutting them in half doesn’t dissolve enough.hellbennt2013-04-17 17:42:02Addding Feb 28th: this is from Kristin F (Facebook): “My LO preferred the cc when we first started but he was extra gassy. He does not like it with mcs (cvs brand) but does like cherry and orange tums. I have 500 mg tums so I crush 2 in the pill crusher. I keep mine room temp because he prefers it. It’s good up to 7 days that way. I put it in a bottle nipple w/a phazyme gas drop & give it when he’s hungry. It took a while before he would agree to all this. He likes the mcs but only in small doses not as large as I need to give w/zegerid. Good luck!”
Adding: April 2015: This is from Emmy J., (Facebook): “My child MUCH prefers when I make the batch with part tums, part cc powder, simple syrup and grape flavoring. He really hates it with MCS.”

November 16, 2011 at 9:40 pm #69385AnonymousInactiveHi Shelly! I see where your LO was once on Prevacid, would like to hear back from you as to how she responded on this Zegerid mixture. My son is currently on a compounded Prevacid mixture and am considering going the Zegerid route to avoid the 30 min rule amongst other issues!
November 16, 2011 at 10:01 pm #69386AnonymousInactiveThis is Hellbennt, taking over this post:
An important note about buffer needed to protect the omeprazole in Zegerid
Although the 20 and 40 mg Zegerid packets contain different amounts of omeprazole, both packets contain the same amount of buffer. Therefore, the 40mg packets of Zegerid have half the buffer per each mg of omeprazole when compared to the 20mg packets.
So, when using the 40mg packet of Zegerid (which we recommend in children under 2 years of age) you have to add some additional buffer to protect the omeprazole from being destroyed by stomach acid. We have found that Mylanta Cherry Supreme works well as the additional buffer in this setting. It accomplishes four goals:
It is the additional buffer
It improves the taste
It reduces gas
It reduces the likelihood of the child spitting up the dosehellbennt2013-06-10 19:13:45
November 17, 2011 at 9:31 am #69389AnonymousInactiveI started a Facebook Group page called “Fans of Marci-kids and Reflux Solutions.”. I’m hoping this can be another way to communicate the knowledge from marci-kids and share information.
November 19, 2011 at 7:14 am #69398AnonymousInactiveFor you guys switching to Zegerid (i’m about to too), I read somewhere you have to add additional omep if using 20mg (because using 2 20mg would be double the bicarb..too much.. Does that only apply when using the Rx Packets? not OTC capsules??
November 19, 2011 at 10:31 am #69399hellbenntKeymasterI think there are two seperate instructions?
I will call them A & B:A) Zegerid – OTCDr. P says that he prefers the following, because of the amt of bicarb:(1) 20mg Zegerid Capsule plus (1) 20 mg omeprazole [prilosec] capsule plus 19ml mylanta Cherry Supreme. Shake and refrigerate, good for one week To help with excess gas from the buffering in Zegerid you can add Mylicon drops in the mixture and some in the bottleOk, straight from Dr P, regarding OTC Zegerid capsules: (1) otc zegerid capsule + (1) otc omperazole capsule + 19mls mylanta cherry supreme [mcs], mixed at a concentration of 2 mg/ml.He then said: “19 mls [of mcs] is plenty, but 13mL [of mcs]& the remainder in water is fine too.”Then, I said the following & he agreed: Basically you use enough calcium carbonate in whatever form (mcs, tums, cc powder [NOW brand]) to equal 1,000-1,500 mgs of calcium carbonate and add enough water to make it 19mLs. This makes a concentration of 2 mg/ml and 3.5mL is the minimum volume to give each dose.I am now adding (to keep everything all together): “Shake & refrigerate; good for one week. To help w/ excess gas, from the buffering in Zegerid, you can add Mylicon drops in the mixture & some in the bottle.” Also adding that some have had success w/ Little Tummies Gas drops (right??). whew! I think we’ve got itFrom Dr. Philips:both are 28 days stable in frig – this means both mixtures, whether it be using RX zegerid packets or using OTC mixtureB) Zegerid RX you need the 40mg packetsThis is how Dr. Phillips is saying to mix the zegerid 40mg packets: Zegerid 40mg packets, add 6.5mL tap water to 1 packet. This will make 10mL (40mg in 10mL), Do this in an oral dosing syringe (20mL size syringe preferred).
The add 10mL Mylanta Cherry Supreme to the syringe. Now you will have 20mL total volume and 40mg omeprazole. (2mg omeprazole per each mL)
This is stable in the refrigerator for 2 weeks. Shake before each dose. You can put the dose in the bottle of milk. Depends on age and weight. Must use at least 3.5mL per dose, three doses per day or don’t waste your time. Alternatively, use a Tums QuikPack in place of the Mylanta Cherry Supreme – then add enough water to make 20mL total volume.found this:Tums is an excellent alternative as it does not contain magnesium hydroxide (that is what causes gas and loose stools)
40mg added to 6.5mL tap water in an oral dosing syringe (20 or 30mL size syringe) then add one tums quick pack or smoothies. Then ad enough water to make 20mL total. Then after all this you will have 2mg of zegerid (omeprazole) in each mL. You can give it right before a bottle too.
Originally posted by WillowBug 03/10/2011 http://community.babycenter.com/post/a24777581/how_to_mixing_rx_zegerid_40mg_ratiosI’ve seen a lot of posts asking for this – so here it is: (I just had Dr.P check the math.) Dr.P said he prefers LOs on 2mg/ml, but as we all know, every LO is different – so do what is best for your LO & of course what is in agreement with your Ped or GI (whoever is Rxing it). A few things to remember…
1. The total volume is the key to get the right ratio.
2. MCS or Tums or pure Calcium Carbonate powder needs to be used as a buffer. Make sure you use the exact brand name listed in the hyperlink. These have all been tested to be lead free.
3. Following mixing and dosing instructions on MARCI-Kids & of course what is in agreement with your Rx Dr.
1 Rx 40mg Zegerid packet + 16ml water + ¼ tsp. CC Powder (total 20ml solution) = 2mg/1ml
1 Rx 40mg Zegerid packet + 9ml water + ¼ tsp. CC Powder (total 13ml solution) = 3mg/1ml
1 Rx 40mg Zegerid packet + ~7.5ml water + ¼ tsp. CC Powder (total 11.5ml solution) = 3.5mg/1ml
1 Rx 40mg Zegerid packet + 6ml water + ¼ tsp. CC Powder (total 10ml solution) = 4mg/1ml
Stability: 28 days when refrigerated, 7 days at room temperature*
Prior to use, shake well.
Between each use, keep refrigerated.
TIPS:
Each LO is different – choose your buffer according to his/her needs. For us personally, we have tried all 3 buffers and we prefer to use the Pure CC powder. There is no grinding (like needed for Tums), no flavor or additives & the final mixture is very smooth.
I do not level the measuring spoon when scooping my CC powder. I do slightly more than ¼ tsp. because this is what we have found to be the best for our LO. You can use up to ½ tsp per packet – so a little more than ¼ tsp.
I also suggest using gas drops in.every.bottle – especially in the beginning. We have tried multiple types. Little Tummys is the best for us. (Make sure it is gas drops – not gripe water.)
A NOTE on buffers:
The above ratios are using the CC powder, but you can adapt the ratios using a different buffer if you chose.
· ¼ tsp. of CC powder = ~750mg of calcium carbonate
· 1 extra strength TUMS or 1 TUMS QuikPac = 1000mg of calcium carbonate
· 1 TUMS Kids Smoothies (contains wheat & dairy) = 750mg of calcium carbonate but often have a better flavor for LOs.
· Refer to MARCI-Kids if you are using MCS
***I hope this helps answer questions and makes posting easier. I’m happy to change this post if there are errors.

November 22, 2011 at 11:21 am #69430AnonymousInactiveSo, this mixture only contains 40mg of zegerid in it. I think that would be less than two days’ worth of medicine on my 1-year-old’s current dose. Does anyone know how much medication is in each ML of this formula? I’m not sure how much to give her after I’ve mixed it.
Can’t believe caracream-z is gone. It’s the only thing that’s helped our daughter. I kept hoping she wouldn’t need it when she was walking, but she still does!

I called my pharmacist and she said the 20 mg omeprazole capsules (prilosec) do not have sodium bicarbonate in them.
Joel in one of his previous posts (that I believe is stickied) said, “The 40mg packets of Zegerid contain proportionally half the amount of buffer as the 20 mg packets, so when using the 40 mg packets, mixed at a concentration of 2 mg/ml, a minimum volume of 7ml (14 mg) should be administered per dose.” So would this 7ml volume be required for the 20mg OTC Zegerid capsule plus 20mg omeprazole capsule cocktail?
November 28, 2011 at 1:13 pm #69468hellbenntKeymastergood question…I will try to find out the answer to this
found out the answer:
(1) otc zegerid capsule + (1) otc omperazole capsule + 19mls mylanta cherry supreme [mcs], mixed at a concentration of 2 mg/ml. Also: “19 mls [of mcs] is plenty, but 13mL [of mcs] & the remainder in water is fine too.” Basically: you use enough calcium carbonate in whatever form (mcs, tums, cc powder [NOW brand]) to equal 1,000-1,500 mgs o…f calcium carbonate and add enough water to make it 19mLs.
