I haven’t personally seen dosing over 10mg/kg/day in infants before, but Zantac is an extremely safe medication so I wouldn’t expect any negative effects. Beth is right, the maximum total daily dosage for children 1 month to 16 years in the treatment of GERD is 300 mg, and in the treatment of ulcers is 150 mg. Either way, you are still below the maximum total daily dosage recommendations.
My only advice is that if you find you need this high of a dose for a prolonged period of time, it might be more beneficial to switch to a PPI. But in the short term, I don’t think there would be any problem.
