HeLp – Nutramigen making baby worse

My son (3rd child) was diagnosed with reflux at 3wks old. His Pedi started him on Zantac, but there was no change. (Note: He arched back, cried before/during/after feedings, has congestion, spit up and swallowed back down) At 7wks he went to a CHOP GI Specialist. She put him on Prevacid-compound (1/2 tsp or 2.5 ml). He was good the first 3 days, but hasn’t been since. He is now 13wks and his crying has gone down from 6-7 hours daily to 4-5 daily, but he still seems to be in pain and uncomfortable. Giving him the compound every morning on an empty stomach and not letting him eat til 30 mins after. I try to get him to take all of it, but it is a battle as he always spits it out. He was breastfed and even with a strict diet (dairy and soy free) he did not change. He had the barrium swallow and it showed reflux but pipes were clean. He went to GI again this past week and she has him trying Nutramigen for a week. He is on day 5 of the Nutramigen and seems to be worse. He spits up all the time, has diaper rash, takes 2oz cries calms down and takes another 2oz, and is arching his back again. He is scheduled for an endoscope on April 2.

I am at my wits end and don’t know what else to do to help him. I don’t know if it takes this long to see a change once on Nutramigen or if it’s posible he is reacting bad to it (Note: he would not take Alimentum at all). GI says the next step is Neocate, but that is super expensive and there is no definite that he has a food allergy, no blood or mucus in stool.

Sometimes I wonder if this will ever end…

Have you checked the marci-kids dosing calculator? I wonder if his prevacid dose is too low. My son needed 30 mgs per day (of the solutabs) before he got any relief.

Check the bottle to see how many mg’s per ml of Prevacid he is getting and then check out this thread about PPI’s.

I started giving him 3.5ml of the compound (my insurance wont pay for the solutabs) and it helped the first day, but not since then. But, I think the Nutramigen is affecting him now. He has been on it since March 18th and has an every-other-day pattern of good days and bad days. Then we were unable to find the regular Nutramigen and he has been the worst since starting the new Nutramigen with Enflora. He has more testing on Monday (endoscope), but I am calling his GI to find out what to do about the formula. She said the next step was Neocate, but I don’t think he has a food allergy as the hypoallergenic is making him worse.

you should TELL doctor and/or pharmacist HOW TO COMPOUND the med.

I did research the stability and what to tell pharmacy. She said they only have one mix and it doesn’t change. She also said its stable for 14days, but every site I researched said 5-7 days. I’m trying to get insurance to cover prevacid tabs…

Turns out he is cranky not just from reflux… he has a food allergy. Just had a sidmodocopy (spelling is off) and the biopsy says he has an allergy. Going to start him on Neocate tomorrow, $150 out of pocket for about 8-9 days, then WIC should be able to help.

Lets see how he does on this new formula and the prevacid.
