Thanks Karen. Sorry about your bad night. Sounds like she really needs a Prilosec increase. Good luck Monday. (If your’re worried about talking to the doc about her meds, try role playing with DH. It really helps me when I’m up for something stressful). Please let us know how it goes.

First of all, let me say how absolutely annoying it is to get schleped off on the NP and never even get to see the doctor. That could be my fault and I never checked to see who our appt was with – I just assumed it was with the doctor. This is only the second time we’ve been to see him and the first time we spent 95% of the time with the NP and the doctor came in for 2 minutes and restated everything the NP said and then left. I figured we might be in for more of the same today but no, this time we didn’t see him at all.
Ok, on to the actual appt… DH and I were pretty certain that Tess’ problem is the instability of the compounded prilosec + flavoring. Yes, I was aware of this problem at the outset but I guess I figured we’d give it a try anyway. Then by the time we figured we knew what was going on this appt was only days away and we thought we’d just grin and bear it until we could see the doc and ask about Zegerid. So anyway, I mention the stability issue to the NP and she doesn’t know anything about that (and clearly doesn’t buy it). I ask about Zegerid and she tells me how the doctor has prescribed it for some people but they really prefer to use the other meds that have been tested for infants (hello?! Zegerid was basically invented for infants!). Whatever. I ask if she can just give us an RX for the prilosec that is for 1 or 2 weeks worth so that we can get a fresh bottle before it starts to go bad. She can’t fathom why we would want to be going to the pharmacy so often and paying more co-pays (um, because my kid is miserable???) but she finally humors us and writes the new script. GEEZ! All we were asking was to write the same stupid dose that we have now but for 1 weeks supply. How hard is that???
She said the doctor would want to do an endoscopy before agreeing to increase Tess’ dose (which we weren’t even asking for, BTW!) so we’re going to schedule that. In the meantime we’ll try getting small amounts of the prilosec so hopefully it won’t go bad before we’ve used it up. I’m also trying to get my pharmacy to get us Cara-cream but I can tell that’s going to take some work. Obvioulsy if using the freshly compounded prilosec or cara-cream does the trick for Tess then we can forget about the endoscopy but since it will be 3-4 weeks before we can get in for that procedure we’re going to go ahead and schedule it now. As much as I hate to have to put Tess through that I would like to know for sure what the extent of her reflux is. We’ll see.
So we pretty much got what we wanted out of the doc for now but it’s amazing to me how frustrating it has to be. Even when you’re making such a simple request (i.e. give me the same thing you gave me last time only in smaller increments).
Well, Tess is starting to wake up from a too-short (unmedicated) nap. We can’t get the new bottle of prilosec until tomorrow. Wish me luck until then.

I hope the smaller amts of Prilosec do the trick.
Do you already have an appt for and endo? Or, are you going to see if the Prilosec works first?

We went through months and months of the same thing until something finally clicked (or snapped?) inside me (maybe the sleep deprivation) and I started fighting back. As for the Zegerid, I know this sounds persistent, but I would print off the info from the website, also the marci-kids website and then fax it into YOUR DOC (not the NP) with your request. I would also write a kind but firm cover page explaining your request, and that Zegerid is omeprazole, so it’s the exact same thing as giving her prilosec, except you don’t have to worry about the stability issues. Then I would tell him that you’ve done your research, and you’ve provided him some of the key information. I would tell him that he can send the script to your pharmacy, give the fax number, and then thank him in advance. I know it’s easier said than done, but sometimes if you drive them crazy enough, they’ll just do what you ask to get you off their backs.
I wish things would getter for you.

She clearly isn’t up on the latest stuff involving reflux and she spouts old wives tales when it comes to other parts of infant behavior (like they’ll sleep through the night better when you feed them solids – hello? every sleep book out there says there is NO research to support this!).
We are making the appt for the endo regardless (just waiting for the scheduling people to call me). If the fresh meds fix the problem I may still go ahead with the endo. If things turn bad again in a couple of months (heaven forbid!) then it would probably do us good to have already done the endo. I can wait until the last minute to cancel it so I’ll just schedule it now and see how things go. I’m a wee bit skeptical that her current dose, even with fresh meds, is perfect. Whenever we have a new batch of prilosec she’s still not that content but it’s certainly much better than the weeks that follow. I guess we’ll just see.
I would push the Zegerid but the NP made it very clear that the doctor would only prescribe it AFTER an endo so I don’t think there would be much point to it. She did say we might try the prilosec capsules which would get around the compound but we need to get Tess eating more solids first (which shouldn’t be a problem I just haven’t moved past cereal yet).
I honestly feel that doctors should not be allowed to practice any kind of specialty without first having do deal with that particular affliction first hand. So if they are a pediatric GI then they should be required to live with a family who has a reflux infant for a month so they can see what it’s like. Then maybe there would be fewer doctors saying “This dose is more than adequate. She’ll be outgrowing the reflux soon anyway.” Morons.

a good idea to schedule the endo and like you said, you can always
cancel it. Hope they can do it soon so you can maybe get a med increase. -
