My name is Laura and our little eight week olds name is Benjamin. I also have a 23m old who was on prevacid until he was 1. You said wits end…i am beyond there.
B was born 3 weeks early. Nicu three days for breathing issues.
At 3 weeks, I found a small dot of blood in his stool. He was being super fussy at night as well and wasnt napping well anymore. We were using good start and no spurting up. Went to the pedi, he found blood through out stool, given nutramigen. He didn’t eat for almost that whole next day and was now screaming at night, and during the day, basically when he wasnt sleeping. 20 minutes after every bottle he sneezes a ton, cough, gags, can’t catch his breath… Just a mess.
Called pedi 2 days later in tears after only sleeping 1 hour total those two nights. They switched me to alimentum, which he’s been drinking fine. Also rx of zantac. That night, has horrible gagging spell, screams for two hours, turns blue, we go to er. He sleeps the whole time we are there, they do an US for intususeption, nope, and send us home.
Over the next few days, he’s screaming non stop in pain, turns blue again, back to er. Dr. Tries to tell me that maybe he has a hair around his toe or penis. Seriously!? While there he starts spitting up, arching, the whole shebang. Dr. Finds two corneal abrasions, rx prevacid for reflux, sends us on our way.
Screaming continues for 5 days and nights. Spitting gets worse, and once again turns blue, off to er. While there turns blue again, cries hysterically for 6 straight hours. They put an oxygen monitor on him… Goes to 49% a few times whole crying, no one seems to mind. Dr. Orders another us and a chest x Ray, all come back fine. Sent home still on 1.7ml if prevacid and told he has colic.
Make an appt. With a gi dr. Get in 5 days later. Told to up dose of prevacid to twice a day, given levsin drops for suspected spasms, told we may need to try rice cereal in bottles, and there is still blood in his stool after being on alimentum for three weeks. we schedulEd an upper gi scan on Monday, and have to take a stool sample. they also did a cbc and found elevated platelets… They didnt seem too concerned. Said wed test in a month. Called mypedi and told her how worried I was. She calls childrens and gets me an appt Monday after the scan with the head gi at Cincinnati childrens! but I want to be prepared. He’s still screaming, not sleeping much, spitting, gauging, choking, stopping breathing, arching. I gave him those levsin drops against my better judgement tonight and they did nothing! He has slept 2 hours all night. I also think he may have sandivers syndrom…i have video.
Anyway, this is a partial vent and partial “any ideas?” post. He is in obvious pain and something isn’t right. What should I demand from the dr? Questions I should ask? Thanks so much!

OK: this is what you demand (I love your attitude, lol!)
the formula isn’t working- every formula change needs to be given a full 2 weeks, or even three weeks, to see it’s full effect (in fact, you will learn that EVERY change takes two weeks. frustrating, I know!).
I would now change to neocate.
forget the rice! that is so old fashioned!IF your baby needs thickening then it’s because they’ve administered a Swallow Study and THEN the results from that give SPECIFIC consistancies of how/what to thicken & you’re given a thickening agent.
Adding cereal to an already sensitive digestive system is not a good idea, at least in MY book- but you must research all you can and advocate for your baby- I cannot…

LEARN all you can about the medicines- the dose of prevacid sounds way too low and not effective- the hard thing is that the product that can help you is smack dab in the middle of being revamped, so all I can suggest is to get on the waiting list – you’ll know what I’m talking about in a moment- I’m Laura the Linker, ha! – I have a TON of links to give you- anyway there’s a way to make the PPIs effective and it’s from a product that used to be offered by the U of Missouri but the University stopped the funding, so a private company is starting production but they are still working out the kinks and it’s frustrating- anyway read all about it here:Prevacid 101:

zegerid has the drug prilosec in it and it has buffers, so that it can be given w/ food. Otherwise, you must give a PPI on an empty stomach, followed by a ‘meal’ 30 minutes later. Examples of situations where you have to follow the ’30 minute rule:’ if you use prevacid solutabs or, if you use either prevacid or prilosec capsules (you would open them up and give the beads inside the capsule- you would give on an empty stomach and administer the beads w/ 1/2 tsp of applesauce & then you would follow w/ a ‘meal’ 30 minutes later).
why should you care about zegerid? because you can give the medicine at any time, even with food, and you don’t have to worry about timing it around an empty stomach.
are there other options?
yes, you can buy over the counter capsules and administer the beads inside the capsule.
any other options?
you can try and TELL the pharmacist EXACTLY how to compound (make into a liquid) your prevacid or prilosec. However, for whatever reasons, it can be difficult to get a high enough dose prescribed and, for whatever reasons, pharmacist insist they’re doing it correctly their way and they don’t take kindly to compounding the way you or your doctor tells them to.

So the gi I am meeting with is: http://www.cincinnatichildrens.org/service/n/neurogastroenterology/default/ his name is listed as the director. This makes me a bit nervous… Although I have attitude 😉 I want to bring up that 1) I don’t feel comfortable giving the antispasmatic drops 2) I want to suggest the compounding issues 3) I’m still really concerned with the blood in stool 4) I don’t want to use cereal 5) i don’t think he really has a milk protein allergy 6) suggest sandifers and show him video…. Is this too much? I don’t feel well versed and will be going myself since my husband has already taken off too much. I’m just nervous that I’ll be stepping on his toes, and he’ll Judy “go through the motions” like every other doc weve seen so far. I am also afraid hell say we haven’t given the two doses of 1.7 ml of 3mg suspension to work… It will have only been 5 days at that point. I have no more work time off until June (I’m a teacher) so this appt really had to count ;)Mrsfranz2012-03-03 11:19:23

I believe that neocate is the next step to help resolve the blood in the stool? and YES use SANDIFERS as EVIDENCE! you’re right about maybe being told to give the compound a chance but: tell him to please HUMOUR YOU and is he aware of research from U of Missouri regarding PPI dosing of infants?if you go to the Medicines Forum and look at the stickies (posts ‘stuck’ at the top so they don’t get lost), there are articles and everything to site…ask him about zegerid and if you can get a rx for it, get the 40mg packets- again, tell him to please humour you- tell him you won’t use the whole packet (yeah right) but you’ve researched about the buffers, etc…and/or TELL him to humour you and to write the RX for the compound to be made SPECIFICALLY how you tell him to – I wouldn’t necessarily say you got this ‘off the internet’ – it’s up to you- perhaps a friend/relation/colleage (I’m a school counselor, LOL!) spoke to you at length about all of this…March 3, 2012 at 12:08 pm #69985AnonymousInactiveDr. Kaul… He runs the motility disorder program. is that OK?
