- So we had a gi doctor appointment today. My concerns:
1. We discussed the Sandifer’s – he didn’t seem to know what it was and didn’t really seem to care about it since she hadn’t had an episode in a while and the prevacid seemed to be helping. She has had one episode in the last 3 weeks and it was when dh forgot to give her her medicine.
2. Apnea – again, didn’t seem to care all that much. She is on a monitor after turning blue and being rushed to the ER. We have yet to have it looked at as she has only been on it 3 weeks.
Basically – he feels (and I agree) that the reflux is being pretty well controlled with the prevacid. But I am concerned about the form of the prevacid (compound) – but he didn’t seem to know a lot about this either.
Then we get to the MSPI issue . . .
I stressed 3 times that she is miserable 70% of each day, that she has a lot of abdominal pain and discomfort, what her sleep is like, etc. He acknowledged that he thinks she is MSPI but felt the Alimentum was helping, again “enough”.
He basically felt since it was better on the Alimentum that we should just stay on it. I told him that a friend had sent me Neocate to try for her and he said it, “wouldn’t hurt” but he thought she was doing okay on the Alimentum. He felt the reflux was complicated by the MSPI but again, the Alimentum is helping “enough”.
I then asked about the probioitics and he said he didn’t really know anything about them . . .
So, if I try the Neocate and it is better I need a prescription to get my insurance to help cover the cost. I am not really happy with the whole she is doing better so let’s stay where we are at thing. Yes, she is better – but better is 70% miserable as opposed to 100% miserable. I think that she deserves to be in less pain and discomfort and to have more opportunities for comfort, happiness and growth (developmental). He is like, well she is okay because she is gaining weight. Of course she is gaining weight, part of the way she copes with the reflux pain is to eat and soothe her throat.
The only thing he did do was order a swallow study because both Tim and I mentioned that she gags a lot. But he left it up to us whether we wanted to do the swallow study or not – so we said yes. She had an upper gi and it showed that everything was fine.
I say we need a new gi, Tim was okay with how the appointment went.
My thoughts:
He doesn’t know about some really important things (the sandifer’s, probiotics)
To me, better is not good enough – our goal should be maximum comfort and I don’t feel that he has explored all of our options
And, do I try the Neocate or not? If it improves her hind end issues, then what do I do about continuing to get it?
And we should definitely get a new ped gi, right?

January 24, 2009 at 9:51 am #60532AnonymousInactiveok, well, my toughts….. probiotics is more of an “eastern meds” approach rather than a “western meds” approach. I have yet to have a doc recommend them. I was told I could use if I wanted to, and we did, but didn’t come from the doc.
2nd, I would try the neocate because if it helps and you report that to the doctor he seems like he would be willing to write the script for you.Believe me, totally sucks to feel invalidated, but i think you can get what you need from him and no matter how wrong it is, we do have to take the lead moreso than the docs sometimes.We used compound prilosec with Carter with much success as we did fill it every two weeks and used a pharmacy that did it correctly, so if it is working, it could be fine.Hang in there! others may feel differently, but i would stick with him for now….January 24, 2009 at 3:44 pm #60534AnonymousInactiveWith my older son our peds dr. didn’t even want to refer us to a GI because “he was gaining” even tho it was such a HUGE struggle to make him eat. But eventually I left her no option and demanded a GI and low and behold he had dysphagia and severe GERD.
WIth the new baby again my peds dr. said ” but he’s gaining” but when I told her how his feds go she was all to happy to refer us out to a peds GI. At first I didn’t think it was reflux but since he has been on preacid for a week with thickened formula he’s eating 5 ozs at a time.
ANyway- if you are unhappy speak up and go see someone else. I would try neocate for a good 2 or 3 weeks (transitioning sloowly, if you find baby doesn’t take to it) and if the transition seems good, to maybe try adding in some probiotics if you think there is still gastro stuff going on..if baby is on marci-kids dose of prevacid then stick with that or, if you feel it could be increased, then after switching to neocate fax the dr: first thanking him then making short & sweet statements- stating you’ve switched to neocate, please write the letter for the insurance, thank you, prevacid is going ok, but due to weight gain it is time for an increase, thank you…also if you’re concerned about the effectiveness of your compound, you can ask dr for rx for for prevacid capsules in the fax as well…and order bufferbabies from refluxsolutions…you’re doing a great job! hang in there! we’re here for you!hellbennt2009-01-24 22:13:40
