Ok, here is the short version of our story with reflux. I have a 3.5 yearold that had silent reflux as an infant that resulted in esophagitis, we had to have an upper GI the works, she was put on Prevacid at 3 months (the older form you crushed and put in her bottle once a day) it helped her nearly immediately and she was weaned off at 10 months and had no further problems, book closed. 8 weeks ago I had my 2nd child a precious little boy and we recognized the symptoms at 3 weeks and stratred him on Prevacid solutabs 15 mg once a day. I know after visiting and emailing the MARCI site that this is not a large enough dose. He is gaining plenty of weight thank The Lord, but still coughs and gags during burping/refluxing while he’s eating. We have run the gamet of Enfamil formulas and are currently on Nutramigen with little help from it. I’m desperatley looking for any advice on combining meds. or different formula that may be beneficial. I’m going in for a re-check with my ped. tomorrow and would love to be able to discuss any helpful advice. Thank you so much in advance for any help, I know you all can sympothize.

That would concern me. You could possibly be dealing with aspirations. I’d talk to the GI and let them know this is happening so that they can let you knwowhat to do.September 8, 2008 at 1:37 pm #56244AnonymousInactiveAnother formula option if you aren’t happy with the Nutramigen is Neocate or elecare. These are totally elemental formulas that don’t have any milk/soy protein in them. Many babies with reflux also have MSPI (milk/soy protein intolerance). We did okay on Nutramigen (it still has milk protein, just broken down) but other babies need the neocate/elecare to help. Are you giving the prevacid on an empty stomach followed by a feeding 30-60 min later. this is really important. how long have you been on it. Are you giving 1 dose or are you breaking into 2 doses, these babies metabolize the meds so quickly. Do you have any Zantac on hand? You can give that as well just spaced 4hrs apart from the prevacid. Another option that doesn’t require a prescription is Cherry Mylanta Supreme.. you can give ~1ml up to 5x/day.. it can however cause loose stools… I think you may need to give it 4hrs apart from the prevacid as well. Have you had a swallow study done? If you baby is coughing choking during feeds you may have some aspiration going on and then you would have to look at thickening the feeds as well.. espicailly if you go with the neocate/elecare as they are very thin and many babies tend to spit up more inititally as they are getting used to thinness of them. I know that is allot. Also check out Laura’s groupie intro stickied under the introduce yourself topic… it has tons of great info. Glad you found us.
September 8, 2008 at 4:23 pm #56256AnonymousInactiveIt seems like everybody loves Neocate around here….including me! My baby also didn’t do well on the prevacid solutabs because of the MSPI issue. Maybe try zegerid?
September 8, 2008 at 4:25 pm #56257AnonymousInactiveHe doesn’t exhibit and symptoms of a true milk allergy, our ped. just suggested it would be the easiest on him. We are giving him his Prevacid 30 min. prior to a feeding, I started giving him 7.5 mg in the morning and a whole 15mg at night 2 days ago, but so far has not helped. To make matters worse I can’t find and of the antacids that the MARCI site recommends in my small town (pulling my hair out) so I have no idea if I can use another version of them safely or not. I have not given him and Zantac as I’m unsure how that work with a PPI, I know that are an H2 rec. inhibitor so I was worried about combining the 2. We have not had a swallowing study done yet we are waiting to get in with a ped. GI.
September 8, 2008 at 5:30 pm #56262AnonymousInactiveYou may end up having to order your Cherry Supreme Mylanta off of the web. Also, make sure that you are giving the Prevacid on an empty stomach. Very important as well as the 30 min before meals. You can give both Prevacid and Zantac together, you just have to make sure they are spaced out 4 hours from each other.Since you are going to your pedi, I would make them do a fecal hemoccult test on his stool. This test checks for microscopic blood in the stool, which can be a sign of intolerances. I also 2nd the upper GI/ swallow study. (prilosec) We cna’t do prevacaid due to Jedd severe oral issues or we woudl prob. switch.
You can askthe pedi or GI for an RX for Carafate. It’s like super mylanta that is taken 30-45 mins before each meal. It does really well.The hard thing to understand is the fact that it can take a full 2 weeks to see wha the meds are doing. Even then if there was damage you have to wait longer. It takes sweet time () for the meds to hela lthings up and see results.I don’t know ho wbig yo uLI is but Jedd takes 5cc of Prilosec and 1 cc of Axid. He * can* take up to 10cc of Prilosec and 2.5 cc’s of Axid per day.We have him weaned rightneow and are in a holding pattern. SHeesh it only took 22 months to get weaned. We are n’t planning on weaning any further BTW. We will do that when Jedd cna verbalize how he feels.September 11, 2008 at 11:04 am #56396AnonymousInactiveOk, we saw the ped. yesterday and are scheduled for an upper GI topmorrow morning. He upped his prevacid to 15mg twice a day for a total of 30 mg and added zantac 1 ml once a day. My question is do I give prevacid in the am then zantac midday and then prevacid again and bedtime, or prevacid prevacid and then zantac at bedtime????

Glad you got the increased dose… sounds like the ped is reasonable & willing to work w/ you! Good luck w/ the Upper GI tomorrow.

My poor little boy looked at me like I was pouring battery acid in him 🙁

We have the cherry supreme mylanta that was recommended, my wonderful mother over-nighted us some from Houston. So do you actually mix the zantac and mylanta together and then draw them up and administer them???
September 11, 2008 at 4:20 pm #56434hellbenntKeymasterthat’s what I did. I used a little medicine cup or even shot glass and mixed them up. I liked to use the 1ml syringe bcse I’d slide it in far along inside of cheek and squirt a little, run syringe over back of tongue and squirt some more- back & forth.
