My DS Oliver was diagnosed with reflux at 1 week old. He’s been on Similac Advance, Good Start, Good Start Alsoy, Isomil Soy & now Alimentum. He’s been on Alimentum for 2 weeks & at first things seemed to get a bit better…he was vomiting less [he used to projectile vomit on all the other formulas], sleeping better etc. but we feel that was a definite honeymoon period, as over the last 3-4 days he’s reverted back to how he was on the other formulas :/ Because of some of his symptoms [stomach heaving, wheeziness, relief after vomiting, loose green stools, bad/smelly gas, wet burps etc. we’ve been wondering if he has MSPI.
He’s 4 weeks tomorrow & he has a doctors appointment to see if the Alimentum has worked thus far. We’re going to ask specifically about making making a switch to Neocate or Elecare, but also about meds. At the last appointment Zantac was brought up, but the doctor said she he was too young…we now know that’s not the case. We’re hoping she’ll prescribe something, or at least refer us to a pediatrician or a GI specialist…but if she does prescribe something, my questions are:
001. Should we try Oliver in Zantac first or should we flat our ask for a PPI? I’ve read SO many people on this site & others state that Zantac did NOTHING & they had to move on to a PPI. Is it better to start off with something like Zantac, to see if it works? is it dangerous to jump straight to a PPI?
002. If she does prescribe Zantac, what dose is corrent? I think I read 5-10mg/kg/day, split in two doses? So, if Oliver is 9lbs, it’d be around 20mg per day, so about 10mg per dose?

September 1, 2008 at 11:47 pm #55980AnonymousInactiveMy 2 cents: I would change one or the other (meds or formula) but not both at the same time. If you do both at the same time and if it isn’t working, it will be hard to tell what your next step should be. It’s always better (but HARD) to take care of one thing at a time.
I would try the formula first, but that is my opinion. Good luck and I hope that things start going in a good direction for you guys!!

if & when you try meds I WOULD ask for zantac because then you’d have a prescription! THEN after 4 days you could ask for a PPI and still have the zantac to give while you’re waiting the 2 weeks to see the full effects of the PPI.September 2, 2008 at 10:31 pm #56001AnonymousInactiveWell, that was a bust.
2 weeks ago Oliver’s doctor prescribed Alimentum & she seemed to agree he was having reflux issues. Well, today she basically stated there’s nothing wrong with him & she didn’t feel comfortable prescribing anything.
This was after we told her ALL of Oliver’s symptoms, which we even took in with us written on a sheet of paper…his vomiting, squirming, arching, tummy spasms, wet burps, wheeziness, crying out in pain during sleep, not liking to be on his back…the list goes on & on. She basically said all those things can be explained away & we have a normal, healthy baby. She even said his lack of sleeping/having restfull sleeps & crying might simply mean we have a colicky baby :/ I’ve read enough to know that even IF colic is a real thing, that colicky babies cry WAY more than Oliver.
She seemed SO uneducated or behind the times…she didn’t recommend the use of gripe water because it “has alcohol in it”, but the gripe water we use does not…she didn’t seem to know about elemental formulas…she also stated that MSPI is VERY VERY rare. I’m not sure if I’m correct on this, but I believe it’s lactose intolerance that’s very rare in infants, while MSPI isn’t as rare at all.
She made an appointment for next week & said if we think his problem is persisting, she’d refer us to a pediatrician or a specialist. We’ll be taking her up on that.missnita2008-09-02 22:33:22

you’re correct that MSPI is not rare (why would there be so many formulas?) and that lactose intolerance is (it’s not unheard of though! and the general public must still be confused so that’s why there’s lactose free formula? of course it’s for the genuinely lactose intolerant)September 3, 2008 at 6:37 pm #56028AnonymousInactiveSadly, I live in an area in Canada where doctors are VERY hard to come by. They actually had to assign Oliver a doctor when we were in the hospital before he was even born, from a list of doctors the hospital BEGGED to take on new patients. Changing to another doctor just isn’t possible.
She IS simply a family doctor. You don’t get a ped. here unless referred. But she did say she’d refer us if we felt it neccessary at next week’s appointment.
