This is a good forum for me!!! I need HELP! My dd is almost 4 months diagnosed with silent reflux at one month old. She has been going downhill with her feeds for a couple of weeks now. My ped was treating her with Zantac and Alimentum for MPI. DD started eating less and was referred to a GI dr last week. He switched her to Axid and Neocate. We weaned last week with the two formulas mixed. We started Neocate on Monday. Now she he has dropped from 25 ounces a to 15 ounces a day. I spoke with the GI dr and they said to go back to Alimentum because she might not like Neocate’s taste. She also passed her stool test with no blood. I also took her to the ped this morning and she is not sick. DD is chewing on the nipple. Once I remove the bottle to burp her she cries and screams when I offer the rest of the bottle. Then she chews on her hand like she is still hungry. What could be going on with her? She is on the 3 ml of Axid a day. I give it to her 3x per day. At this point I don’t know where to turn to. There is only on ped Gi practice in my city (as told by my ped). I also signed a form that said I can’t switch drs in the practice. Any suggestions or ideas?
August 27, 2008 at 7:49 pm #55808hellbenntKeymasterhi & welcome!Perhaps find a family doctor? many on this board have found that their family doctors are more comfortable/willing to give doses of PPIs, as marci-kids suggest. a PPI is a proton pump inhibitor such as: prilosec, prevacid, nexium, protonix.http://www.marci-kids.com is at the forefront of research addressing PPIs and infants.

August 27, 2008 at 8:48 pm #55809AnonymousInactiveI agree with Laura that she needs a PPI. It sounds like she is pain, but is hungry too. She is wanting to eat, but it hurts too much. Axid and Zantac, H2 blockers, don’t work nearly as well. I would try for a good dose of a PPI, like Prevacid or Zegerid.
In the meantime, try some Mylanta Cherry Supreme before a feed and see if it helps soothe her throat a little so that she will eat some.August 27, 2008 at 10:05 pm #55817AnonymousInactiveJust to add a tiny bit to the others…Alimentum has trace amounts of milk in it. The only formula to my knowledge that has NO milk in it is Elecare.
August 28, 2008 at 12:40 pm #55823AnonymousInactiveMy dd had a feeding aversion- BF, bottle feeding, and solids. She hated anything to do with eating. A child with a feeding aversion really needs a stronger med, like a PPI, at a high dose, in my opinion. It’s really importatnt to get the pain under control. The zantac family of meds did nothing for my daughter, neither did a low or moderate dose off PPI. By the time we got her meds sorted out her feeding aversion was so terrible that it took extensive feeding therapy and time to turn things around.
How is your daughter’s weight? Do you need to be concerned about failure to thrive, or is she gaining well? 25 ouces a day is quite good. We were lucky if we could get up to 16, so I know how you feel.My advice:1) Check out http://www.marci-kids.com . Read up on reflux and PPI meds. Know what brand and form of PPI you would like to ask for. Also read up on dosing so you know what dose you’d like to try. Print off some information for you GI to read. The fact that your GI is hopping from one H2 blocker to the next without trying a PPI likely means that s/he is on the conservative side of things, and may also dose on the low end when it comes to PPIs- that’s just my experience though.2) Are you able to persist with the neocate a bit longer? Sometimes it takes some time for the transition and for the switch to make a difference.3) Tell us where you’re located. Maybe someone here can recommend a local doc to you.Good luck.August 28, 2008 at 1:25 pm #55827AnonymousInactiveI am located in San Antonio, Texas. I think my GI dr is conservative. He said that she had a MPI and slight reflux. He thought for sure she still had blood in her stools. The test came back negative for blood. He also said he never gives prevacid to babies as young as DD. I have not tried the Neocate again. She did a little better last night on the Alimentum. She ate well until she burped or choked. With the Neocate she wouldn’t hardly eat to begin with. She is still on the charts for weight and height. She has dropped quite a bit in her percentiles but she was chubby as a newborn.
Her Zantac dosage was 1.8 ml twice a day. The Axid dosage is 1.5 ml twice a day. I have been giving her 1 ml three times a day. She weighs 13 pounds. Could the Axid dosage be a problem as well? I have also checked several grocery and drug stores here and I can’t find Mylanta Cherry Supreme. I called and left a msg for my GI dr and waiting to hear back. They are super slow. I have been waiting for a prescription for Neocate since Monday. Quite ridiculous imo! So far I am not happy with their practice.August 28, 2008 at 1:39 pm #55828AnonymousInactiveI always had luck with the cherry mylanta supreme at Target, in our store it it is on a bottom shelf and you really have to “look” for it. For some reason it seems to always be hidden. I agree with the other moms that you need a PPI. You might have to either go to a different doctor or try your ped or a family doc to get meds that will be helpful.

My Chloe was on feeding strike for 3 weeks, she down from 25-27oz/day to 4-8oz/day (which freaking me out!).
She still on Zantac, and now about 2 weeks, and then I asked for prevacid (but the doctor told me to finish the Zantac first).
I don’t know, so I continue with Zantac, and the day ten, she drinking again!
And I wait maybe 4 hours before feeding her again and change the nipple flow to bigger one.
Hope it works!

I totally agree.I know some kids who start on it right away due to track records with siblings. I know of one in particular that was jsut about DX with relfux in the womb due to constant hiccups ect…and the fact that BOTH parents and both brothers had VERY sever reflux.When he was about 4 days old the started the PPI. August 28, 2008 at 10:23 pm #55849AnonymousInactiveI found the Mylanta at Target! I think that was the only store I didn’t check in the past. What is the correct dosage? I gave her 1 1/2 ml. I saw something about 1/4 – 1/2 teaspoon every 4 hours? Is that correct? She also ate 4 ounces after I gave her the Mylanta (that was her normal amount before the strike). Still no word or Neocate prescription from the GI dr. I called three times today and called the pharmacy. She is still on Alimentum and Axid. I feel like I am back to square one!
August 28, 2008 at 10:39 pm #55851AnonymousInactiveYou will know if you have given too much b/c the side effect is loose stools. But a good rule of thumb would be 1ml up to 5x/day. Depending on how often she is taking a bottle, I would give it about 10 min before the feeding and up to 1ml each time. HTH.

Any ideas on how to make the Neocate transition any easier? I have it mixed 1/2 Alimentum and 1/2 Neocate. She will not drink any of it!
August 29, 2008 at 10:14 pm #55890AnonymousInactiveone of our moms Kristin (kdub) had to use Splenda for her little guy. I can’t remember how much but she just recently got him off of it. You might want to pm her since she hasn’t jumped in to get her experience. They also make flavor packets, but I think they are pretty expensive and that formula is expensive enough as it is. HTH.
