Hi guys….my little guy tested positive again yesterday for the C-Diff Toxins. My GI doesn’t want to treat it though, because he isn’t having any diarrhea or mucous (at least that we can see…he is constipated). I’ve heard of lots of babies that have had to have sections of their colon taken out as a result of the toxin damage. I’m afraid this will happen with my baby if they don’t treat the bug. But, I am also not okay just giving him antibiotics that aren’t going to work and further wreck havoc on his little system. I have tried to look into suseptiility testing…but I live in Florida where everybody sends their stuff through Quest Diagnostics, who does not do suseptability testing. Anyone know where this type of testing is usually done, doc office, local labs, or what?
BTW…No idea how to spell suseptability!

July 24, 2008 at 10:40 am #54413AnonymousInactiveOh, also-this may totally not pertain to your sitation, but I’ll share anyway… when I talked w/ both my ped & ped GI the other day about C-Diff they said the reason they won’t just test the stool/diaper for a baby under 1yo is b/c C-Diff naturally shows up in many many stools for infants. Most times it’s ok. They don’t want to treat it w/ antibx unless they need to… so they usually only treat if they get a positive biopsy from the endoscopy.
Again, sorry if this it completely unrelated to you, but it explained to me why we have to wait until the endoscopy next week to rule it out.July 24, 2008 at 1:17 pm #54425AnonymousInactiveOur hospital does the suspectibility testing… Maybe look into the hospital and see if they will do it. You have been dealing with this for so long… I would stop at nothing to make sure it gets cleared up. You guys have been through so much.
July 24, 2008 at 9:16 pm #54453AnonymousInactiveI haven’t checked Lab Corp….I doubt they do it. It seems most of the more local labs do it rather than large chains. I am going to check it out though. Thanks for the suggestion. I have family in Idaho and Utah where every doc office has its own lab. I may do some research there and maybe fly home for a bit. I’m going to try the local hospitals as well. Does anyone know how expensive it is?
I haven’t gotten a 2nd opinion from another GI, but I do have an appointment with a Ped Infectious Disease doc on Tuesday that is supposed to team up with my Ped GI. I’m excited about that because I really like my GI, I just don’t think he deals with the medication of C-Diff babies often (he’s never put a baby on Vanco before). Ped infectious disease docs should have lots of that specific experience. This way I will hopefully have the best of both worlds.
It is true that many babies test + for the C-Diff bacteria. However, it is my understanding most do not test + for the toxins. The bacteria will eventually get weeded out by a healthy GI, but once the toxins are being produced, its a whole new ball game. They cause lots of damage in the intestines and are super hard to get rid of. Sometimes they result in removing pieces of the intestines. They determine damage to the intestines by endoscopy which makes me curious….Erin, did your doc say anything about the toxins? Did he seem to think the toxins were normal? Or was he just talking about the bacteria? If a baby can have toxins and have it be normal (in the sense that it will just go away eventually), I will cry with joy. This is not something I have heard. But if your doc only treats C-Diff based on endoscopy, then he has to be only treating when there is actual damage done to the colon…right? Hmmm…interesting.

The Ped Infectious Disease dr sounds promising. I sure hope you get some answers on Tuesday!No, my dr. didn’t say if he was talking about bacteria or toxins. I didn’t know to ask that or I would have-sorry! He only treats C-Diff based on biopsy from endoscopy. I will check into this today and let you know what I find out. It sounds like your doc might not even treat the toxins unless there is actual damage. I’m going to ask the GI and ID docs about that. Also, I was going to ask how Evan is doing? Are you nervous about the endoscopy? Do you know if they are doing a full endoscopy? I mean how far into the colon are they going to look?

July 25, 2008 at 12:48 pm #54478AnonymousInactiveIf you still can’t find anyone to do the susceptability testing, there are 2 labs that I would recommend contacting to see if they do it. The first if Great Plains Laboratory and the second is Genova Diagnostics (formerly called Great Smokies).
We have done comprehensive stool analysis through both of these labs, and they have done cultures with susceptability testing for bacteria, yeast, etc. Not sure if they can culture C. diff or not, but I would give them a call to see. If you do test through them, what they do is send a collection kit out to you where you collect stool samples for usually three consecutive days, then ship it back to them for analysis.
