Hi All!
Sorry I haven’t been posting as much lately, but I’m back at work now 3 days/week, where I don’t have internet access, and the rest of the time it feels like I’m just running trying to get everything done!
Dagney had her follow-up visit at the GI on Friday. We saw a different dr, as hers was called to an emergency. She was very happy with Dagney’s progress, and had some things she wants us to do over the next 1-2 months.
Prevacid: She’d like us to start gradually cutting out 1 dose/day over the next few weeks, and see how it goes. She mentioned that if there’s no acid in the tummy then the little ones can develop polyps. Dagney is 8 1/2 months old, so I’m thinking this is a good thing, but I’d like some input on how anyone else has done this. (Dagney seemed to have a flare up a few weeks ago, but she seems to be good now.)
Neocate-Nutramigen: She’d like us to gradually move her over to Nutramigen or Alimentum at 10 months. She mentioned starting with 1 bottle/day Nutramigen, and the rest Neocate on the first 1-2 days, then go to 2 bottles/day etc, until she’s on full Nutramigen. I was thinking of doing 1 ounce per bottle per day of Nutramigen so that all the bottles had similar taste and consistency.
Dairy/soy: We need to come back at 1 year at which time we’ll talk about trialing dairy and soy. She did mention that once she starts table foods, we could try using Rice milk (for things like mashed potatoes etc) in cooking, which hopefully will be fine.
Does anyone have any opinions on this? I’m pretty excited about it all, and it all seems fair to me, but any input and pros/cons would be appreciated.

June 4, 2007 at 8:19 pm #35610AnonymousInactiveWell, I think if it was me I’d try weaning the the meds first and not try any formula changes until she does fine med free for at least a month.
As for weaning from meds, I did it really slowly with Myles. He was on 10 mgs three times per day. I started by reducing it to 7.5 mgs twice a day and 10 mgs at bedtime. I did that for a week. Then I eliminated the midday dose for a week. Then I stopped the morning dose for a week. So three weeks into it he was finally on one 10 mg dose a day (at bedtime). I think I stopped that after another week or maybe two. Natalia posted something about stopping PPI’s too fast leading to rebound acid production, so I think slow is best.
So, if you do it this way and then wait another month to trial another formula she will be almost 11 months old when you start the formula trial. That’s what I would do becuase if you change too many things at once and she starts having problems you won’t know if it’s the meds or the formula.
When I trialed new formulas or dairy with Myles I added two ounces to one bottle per day for the first two weeks. Then when all was well I gradually started adding more until he adapted to the taste.

At what age did you start weaning Myles? And when was he finally off of it altogether? Dagney won’t actually be 9 months for another week, but I think starting it now shouldn’t be too big a deal, right? She’s also on 10mg 3 times/day, and I was considering switching to 2 doses/day: 15mg and 10mg (and cutting out the midday dose right away, so my MIL didn’t have to give it to her anymore) OR doing 7.5mg 3 times/day. Your way sounds a little better to me, so that its a little slower weaning, but it won’t shock her little system either.

June 4, 2007 at 9:29 pm #35628AnonymousInactiveIt sounds scary to me. I would do it very sloooooowly. If she wants you to switch from Neocate to Nutramagin, then she must not be worried about the milk intolerance/milk allergy.
I just think when kids are doing well, let it be for a while to let the GI sys heal, but I am no doctor! I would wait 2 weeks between changes so you know what is working and what isn’t.

June 5, 2007 at 6:08 am #35644AnonymousInactiveI started weaning Alana from meds around 8 and a bit months. I halved her dose for 2 weeks and saw no reaction. I then dropped the rest for a couple of weeks but gave her some zantac for a few days prior to bed. Still no obvious flare so we did nothing. She occasionally had a reflux episode (maybe 5 times a week) for another 2 or so week and now we see nothing. All in all we did the wean over about 4 weeks.
I then tried the dairy thing…yoghurt…no luck with that I’m afraid. I’m glad I did it separtely though, if not I might still think she needed her meds.

To answer your question – I started weaning him at 10 months. He was a puker so when he stopped puking at 9 months I figured he was probably done with reflux but I waited a month to be sure. It’s probably trickier to know when to do it with a silent refluxer, but I think 9 months is a pretty reasonable time to try it.

June 5, 2007 at 8:51 pm #35719AnonymousInactiveDagney’s always been a puker, but has pretty much stopped for the last few weeks (right after her big flare up, it seemed to die off really quickly). I’d say for at least the last 3-4 weeks she’s been basically puke free, except for the occasional small spit up here and there. I’m going to do it. I started today cutting back on the dosage, and we’ll see how it goes!! Thanks for all your input, everyone, I’ll keep you posted!!

I don’t think there is a wrong or right way to wean or switch formula or anything…it seems like so many people have done it different ways. I would just do one thing at a time (lower dose, or switch formula) and give it 2 weeks before doing anything else. Otherwise if you do have problems you won’t know which one caused it. Good luck!

I’m doing it…we’ll see how it goes…I’m cutting back to 3 7.5mg doses/day for a week, then I’m going to go to 2 7.5mg doses/day for a week, then down to 1. This may be a silly question, but should I cut the prevacid out completely, then do the Nutramigen, or leave her on 1 dose/day or so just in case?
And THerese, here’s another silly one, when you say that Alana had some reflux episodes, what exactly happened?
June 7, 2007 at 5:42 am #35845AnonymousInactiveWell, I could hear it coming up and she kind of got that look on her face, that “what the heck was that in my mouth” kind of look. Sometimes she would also tear up in the eyes. Once or twice I think it hurt. But I would wait for it to happen again and it didn’t for a day or so. This only went on for a couple of weeks or so and now I don’t see it at all.
