Sarah had an endoscopy last Friday and we received the results Tuesday. The scope showed mild esophogitis in her lower esophogus and all other biopsies were normal. She did have some enlarged lymph nodes her duodenum, but the pathology showed they were normal lymph tissue.
The good news is that there was no evidence of eosinophils which as you know would have indicated some sort of allergenic process. The bad news is that the level of damage is not consistent with her behavior (frequent night wakings screaming in pain and strange shoulder shrugging) given the dosage of Prevacid she is currently taking (Solutabs, 15mg twice a day).
The ped GI is a little puzzled about what to do now and recommended that we continue on our trial of Prevacid with the correct dosing (15-30 min before a meal, which we were not doing for the first 12 mos or so) for 8 weeks and then reassess. She also said that she would like to do an upper GI to look for any malrotation that might be causing pain, but I am very hesitant to do any more testing at this point. I countered with suggesting that I night wean first to see if some of her night waking is behavioral and not a result of reflux pain. Currently, when she night wakes, she screams in pain and the only thing that seems to comfort her is nursing, so I typically nurse her for a while and she falls back to sleep (we co- sleep from about midnight on).
So it’s more wait and see. Two nights ago she actually only woke up twice — once at 1:30 AM and once at 6:30 AM — it was amazing! Last night she was up 4x, which is certainly better than her usual 6- 8+ times. I think I am going to start charting her wakings and her shoulder shrugging and see if I can find any sort of correlation.

When Sarah does her shoulder shrugging, is it one shoulder at a time or both shoulders? Also does she tilt her head toward her shoulder at the same time? The reason I ask is Bryce has been doing a similar movement where he tilts his head to his shoulder. I’m pretty sure it’s to do with his ears, as he’s had an ear infection. We’re taking him in today to see if it’s cleared up yet. He still seems like it’s bothering him & is still tilting his head. Anyway! Just wondering if her shoulder movement was something similar.Good luck with everything. I hope that more time on the Prevacid will do the trick for Sarah.
February 9, 2007 at 9:52 am #24440AnonymousInactiveHmmmm…..that is good news that only minor damage was found and no EOS cells, but confusing that she is still waking so much in pain. Could it just be that she is so used to nursing to sleep that she cannot put herself back to sleep when she wakes up during those light sleep stages that all babies have at night?
Or, I’m wondering if maybe she needs an H2 Blocker before bed. There have been a few moms here who have found that using the PPI meds during the day and an H2 Blocker before bed was the answer for their babies. It seems that even on the right dose of Prevacid or Zegerid there can still be breakthrough acid in the middle of the night.
