We went to see the new ped gi yesterday, he is the fourth opinion. The previous three ped gi said that Brian is taking too much med, and his reflux is under control, and they also relutant to increase the med. After I told the new gi that Brian still burps a lot even hours after he eats, and he wakes up very often during the night. The new gi said his reflux is not under control, so he increase his prevacid to 7.5mg three time a day (total 22.5mg/day), increase his reglan to 1ml three time a day, discontinue the pepcid. After seeking all those gi opinion, I always think that increasing med dose is what I want, but now I hesitate, because I really afraid of any changes. Brian is one now, you would think that he will outgrow the reflux by now, but ped and ped gi said that he may not out grow it for a while due to his low muscle tone. What do you all think? Should I increase the dose? Ped gi said it takes a month to see some effect, does it really take that long (since he is already on those meds, but just different dose)?

Yes, I would go with the increased dosage. 22.5 is still conservative for Brian’s weight (didn’t you say he was 20-21 lbs. before?) and you can’t overdose on it from what I am told. Seth just turned one and is still on 30 mg. He was doing great for a long time but just recently we can hear him refluxing again so I am wondering if he needs even more. The GI nurse told me that teething can aggravate reflux so we are hoping that is what it is and it will pass soon. We go to the GI Tuesday so we’ll see if they up the dosage. Like you though, I was hoping we would be done with the reflux monster by now not getting more medicine but we’ll do what we have to do! Good luck!!

I would increase the prevacid definitely. Has he been diagnosed with DGE? If so, then I’d try increasing the reglan. If not, I might wait on that one.
